Tufting is raised off the surface, so each row throws a small shadow. That shadow is the whole difference — it is why a tufted bed looks layered and considered in a photograph, and why a flat one never quite does.
The diamond-and-stripe pattern here is worked row by row into a soft woven base, and it stays raised through the wash rather than flattening out after a month.

Is it warm enough for winter?
It is a lightweight, year-round weight rather than a thick winter duvet — which is the point for most people, because it doesn’t overheat you in summer. If you sleep cold in January, use it over a duvet or add a layer underneath. If you want a single heavy winter filler, this isn’t it, and we’d rather say so.
What size should I order?

Does the tufting flatten in the wash?
No — it is worked into the base rather than glued on. Machine wash on a gentle cycle and tumble dry low; the texture comes back as it dries. Avoid a hot wash and don’t iron the tufted areas.
